According to <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/7943523\/\" target=\"_blank\" rel=\"noopener\">Rubinstein et al. (1994)<\/a>, The Reverse Lachman Test has a Sensitivity of 63% and a Specificity of 89%.\nFor this reason, it has a moderate clinical value to confirm a PCL Tear, although we have found only one study to evaluate its validity yet.\n<div class=\"embed-container\">\n\n[embed]https:\/\/www.youtube.com\/watch?v=uBFV1Fdtj4w[\/embed]\n\n<\/div>\nTo perform the Reverse Lachman Test have your patient in supine line position with the knee flexed to 30\u00b0. You can stabilize this position by placing your knee under it.\nThen place your contralateral hand on the femur from lateral and the ipsilateral hand on the Tibia from medial like in a normal Lachman test. Both hands are close to the joint line as we want to translate and not angulate.\nThe Tibia must be anatomically reduced in the PCL deficient knee at the start of this test. Rotation is neutral to avoid the recruitment of secondary stabilizers.\nThen exert a posteriorly directed force to the tibia and feel for end feel and the degrees of translation\n\nThis test is positive if the end feel Is soft or absent, all with an increased posterior translation of the tibia compared to the other side.\n\n<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/18219052\" target=\"_blank\" rel=\"noopener\">Lubowitz et al. (2008)<\/a> described a grading for a posterior translation:\n<ul>\n \t<li>Gade 1:\u00a0 1 0-5mm and the tibial plateau remains anterior to the femoral condyle<\/li>\n \t<li>Grade 2: 6-10mm, which correlates with the tibial plateau being flush with the femoral condyle<\/li>\n \t<li>Grade 3: &gt;10mm of translation, which allows the tibial plateau to translate posteriorly to the femoral condyle<\/li>\n<\/ul>\nIf you notice increased translation with the Reverse Lachman test but not with the Posterior Drawer Test, an injury to the posterior lateral corner might be present.\nTo distinguish between a PLC and a PCL Injury, performing the <a href=\"https:\/\/www.physiotutors.com\/wiki\/dial-test\/\">Dial Test<\/a> is recommended.\n\n[cta]\n\n&nbsp;\n\nOther common tests to assess PCL tears are:\n<ul>\n \t<li><a href=\"https:\/\/www.physiotutors.com\/wiki\/posterior-drawer-test\/\">Posterior Drawer Test<\/a><\/li>\n \t<li><a href=\"https:\/\/www.physiotutors.com\/wiki\/posterior-sag-sign\/\">Posterior Sag Sign \/ Godfrey's Test \/ Step-Off Test<\/a><\/li>\n \t<li><a href=\"https:\/\/www.physiotutors.com\/wiki\/quadriceps-active-test\/\">Quadriceps Active Test \/ Active Drawer Test<\/a><\/li>\n<\/ul>\n&nbsp;\n"}},{"acf_fc_layout":"content-content","section_options":{"vertical_alignment":"top","column_right_box":false,"column_left_width":"col-12 col-lg-4 offset"},"column_left":{"content_editor":"<h4>References<\/h4>"},"column_right":{"content_editor":"<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/7943523\/\" target=\"_blank\" rel=\"noopener\">Rubinstein JR RA, Shelbourne KD, McCarroll JR, VanMeter CD, Rettig AC.
