Furthermore, patients report clumsiness and reduced sensation in the hands.\u00a0<\/span><span style=\"font-weight: 400;\">As the spinal cord gets compressed patients may also present with central cord symptoms such as spasticity and urinary retention. It can thus be a potentially devastating condition.\u00a0<\/span>\n\n<span style=\"font-weight: 400;\"><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/22131790\/\" target=\"_blank\" rel=\"noopener\">Cook et al. (2010)<\/a>\u00a0produced a cluster of predictive clinical test findings for a sample of patients using a clinical diagnosis as the reference standard for CSM. <\/span><span style=\"font-weight: 400;\">The goal of the cluster is to detect the disease in stages or to rule out the condition during screening.<\/span>\n<div class=\"embed-container\">\n\n[embed]https:\/\/www.youtube.com\/watch?v=WkRgpMyzQxs[\/embed]\n\n<\/div>\n<span style=\"font-weight: 400;\">The five tests or patient characteristics included in the rule are the following:<\/span>\n<ol>\n \t<li><span style=\"font-weight: 400;\"> Gait deviation: which shows as abnormally wide-based gait, ataxia, or spastic gait<\/span><\/li>\n<\/ol>\n<ol start=\"2\">\n \t<li><span style=\"font-weight: 400;\"> A positive <a href=\"https:\/\/www.physiotutors.com\/wiki\/hoffmanns-sign\/\">Hoffmann\u2019s sign<\/a>: This test is characterized by a reflex contraction of the thumb and index finger when flipping the distal part of the middle finger.<\/span><\/li>\n<\/ol>\n<ol start=\"3\">\n \t<li><span style=\"font-weight: 400;\"> An <a href=\"https:\/\/www.physiotutors.com\/wiki\/inverted-supinator-sign\/\">inverted supinator sign<\/a>: which is elicited by quick tapping near the styloid process of the radius, the attachment of the brachioradialis tendon. It shows in finger flexion or slight elbow extension.<\/span><\/li>\n \t<li>A positive <a href=\"https:\/\/www.physiotutors.com\/wiki\/babinski-sign\/\">Babinski sign<\/a>: This sign shows as extension of the big toe and fanning of the other four toes when stroking the lateral aspect of the foot sole from the heal forwards towards the great toe.<\/li>\n \t<li>Age &gt;45 years old<\/li>\n<\/ol>\n<span style=\"font-weight: 400;\">If 3\/5 of the aforementioned characteristics are positive, the positive likelihood ratio for cervical spondylotic myelopathy is 30.9.
